Online request for promotional offer must be submitted online at http://www.surewest.com/referral/ within 30 days of new customer installation to confirm both parties’ qualification for a $100 credit. The referred new customer will receive a $100 credit 45-days after at least two services have been installed. Please allow 45 days after installation to receive the credit. To receive the billing credit, the new account must be continuously active, in good standing and account paid up until the earlier of the date the credit, or through two full billing periods after installation. The $250 SureWest Visa® Gift Card will be sent to the referring customer within 45 days after the referred new customer begins service at the same address. The referred new customer will receive a $100 credit on their bill after at least two services have been installed. In order to qualify for the $100 credit, the new account must be continuously active and in good standing.
